Volunteer and Board Nomination Forms   To volunteer for Board or committee positions, complete the Board Nomination Form or the Volunteer Participation Form, and submit to the Leadership Advisory Committee. All committee positions are appointed by the Chairman. The Board reviews Board nominees submitted by the Leadership Advisory Committee and presents a slate to the Nominating Committee.
